Why giving clients fewer exercises retains more of them

A long programme looks like careful work. Twelve exercises, four days, an accessory for every weak point: it signals that somebody thought the case through. The problem shows up when you compare what was prescribed against what was executed, because that is where the long programme usually turns into three full days, two half ones, and a week of nothing.

Key terms

Adherence
The proportion of what was prescribed that the client actually does. It is measured by frequency (how many sessions) and by completeness (how much of each one).
Minimum effective dose
The lowest volume that still produces a measurable adaptation. It is not the optimal volume; it is the floor below which the stimulus stops working.
Odds ratio
How much the probability of an outcome changes between two groups. A value of 0.2 means the exposed group had a fifth of the odds of the other.
Dropout
Leaving the programme. Distinct from low adherence: first people comply less, then they leave.

The volume conversation usually happens in physiological terms: how many weekly sets per muscle group, how much frequency, how much can be recovered. That is the right conversation for an athlete under daily supervision. For a client who trains four weeks out of five, there is a variable that arrives earlier and weighs more.

That variable is how much of the programme actually gets executed, and the adherence literature measured it with some precision. The number of prescribed exercises shows up there as a predictor, with an effect large enough to be surprising.

The measured cost of an overloaded programme

The most specific figure comes from an observational study published in BMC Musculoskeletal Disorders in 2009, covering 184 patients with chronic neck or low back pain on home exercise programmes. The authors looked for variables predicting adherence to the prescribed frequency and found one that does not belong to the patient at all — it belongs to whoever writes the programme.

Patients given more than six exercises had an adjusted odds ratio of 0.2 for meeting the agreed weekly frequency, with a confidence interval of 0.1 to 0.9. Put another way: going past six exercises left the patient with a fifth of the odds of complying. The same effect did not appear for the duration of each session, only for frequency.

A much earlier paper, published in Physical Therapy in 1999, had pushed in the same direction with an experimental design: independently living older adults were assigned 2, 5 or 8 exercises, and the group given 2 complied more and executed better than the group given 8. The sample was fifteen people, so the study counts for the direction it points in rather than for the size of the effect.

Neither is a resistance-training study on personal-training clients, and that is worth saying plainly. But both measure the same thing that happens to any client: the gap between what they were asked to do unsupervised and what they ended up doing.

What is actually lost by cutting

Fear of cutting is reasonable if you believe results scale linearly with volume. They do not, and the minimum-effective-dose literature made that reasonably clear.

A systematic review with meta-analysis published in Sports Medicine in 2020 looked for the minimum capable of producing strength increases in already-trained men. The conclusion was that a single set of 6 to 12 reps at 70-85% of 1RM, taken to failure, one to three times a week, is enough for significant 1RM increases over 8 to 12 weeks. Pooling five studies gave a mean increase of around 12 kg in overall 1RM.

That does not make one set the recommendation. It is a floor, not a target, and the authors themselves call it suboptimal. But it changes the arithmetic of the decision: volume you cut still pays partially, while an exercise the client never does pays exactly zero.

A twelve-exercise programme executed at 60% delivers less real stimulus than a six-exercise one executed at 95%, and it delivers worse information on top. How to decide what stays

Cutting without criteria produces a short bad programme. The criteria that hold a cut together come down to three filters, and none of them is physiological.

The first is unsupervised execution. An exercise the client performs well alone is worth more than a technically superior one that falls apart when nobody is watching. Movements that need constant correction belong in the in-person sessions, not in the programme that goes home.

The second is real-world logistics. An exercise that depends on a machine occupied at seven in the evening at the client's gym is an exercise that will be skipped. Asking what equipment is free at their actual training time buys more adherence than any set-and-rep adjustment.

The third is redundancy. A long programme almost always carries two or three exercises covering the same pattern under different names. Dropping one from each pair does not change the stimulus and hands back ten minutes per session, which is exactly the currency adherence is paid in.

  • Keep what the client performs well with nobody beside them.
  • Drop what depends on equipment that is contested at their training time.
  • Drop one from each pair of exercises covering the same pattern.

A short programme demands more precision, not less

A six-exercise programme has less margin for error than a twelve-exercise one: every choice weighs more and there is no redundancy to absorb a bad pick. Cutting and loosening the progression at the same time produces clients who comply with a plan that no longer moves them.

What makes a cut sustainable is precision in what remains: an explicit progression rule per exercise, a log of every set, and a regular comparison of what was completed against what was prescribed. That comparison is the number that anticipates dropout, because adherence falls before the client says anything.

What to take away

The number of exercises is a design variable the trainer controls completely, and the available evidence links it to adherence fairly directly. Six looks like a reasonable ceiling for anything done without supervision.

Cutting works when it comes with criteria: keep what is executed well alone, what does not fight the client's logistics, and what is not duplicated. And what remains needs a clear progression rule, because a short programme without progression stops stimulating quickly.

The operational benefit arrives afterwards. A programme that gets completed generates complete data, and with complete data next week's decision stops being a conversation about how things felt.
